About the Role: Magnolia Health Systems is seeking a Social Services Coordinator to
  • Assist in the entire admissions process from pre-admissions (pre-screening, tours, etc.) to discharge (placement arrangements, homecare referrals, etc.)
  • Provide social services in accordance with accepted facility, professional and governmental regulatory standards.
  • Act as a resident advocate and liaison with facility staff and with community resources as necessary.
  • Develop, as needed and participate in the implementation of an appropriate
  • Behavior Management Program for applicable residents
About you: The ideal candidate would have the following skills and experience.
  • Indiana board certification in social work under IC 25-23.6-5-1
  • 1+ years’ experience in health care setting working directly with individuals
  • Bachelors or advanced degree in social work or a human services field
